Do not treat one good AI output as a finished SOP
Small businesses usually need a lightweight feedback loop more than another prompt. The goal is not to punish employees or automate every exception. The goal is to spot repeat failure patterns early enough to fix the workflow before customers, vendors, applicants, tenants, or owners see a bad handoff.
Human-review rule: any exception touching money, legal terms, hiring, medical/safety advice, refunds, deposits, contracts, privacy, customer complaints, public reviews, or final approval must be reviewed by the appropriate owner or manager before sending or changing the SOP.
Copy/paste exception log columns
- Date / owner: who found the exception and who owns the fix.
- Workflow: lead reply, customer email, review response, SOP draft, meeting notes, invoice reminder, onboarding, or other process.
- Trigger: what input or situation caused the AI-assisted workflow to behave badly.
- AI output issue: wrong fact, missing context, risky promise, unclear tone, wrong next step, privacy concern, unsupported claim, or stale policy.
- Human correction: what the reviewer changed before using the output.
- Root cause: bad prompt, missing input, outdated SOP, weak review rule, unclear owner, tool limitation, or user training gap.
- Decision: keep, fix, pause, retire, escalate, or add to training.
- Next check date: when to confirm the fix worked.
