AI-OUTPUT-CORRECTION-LOG-READY
Why correction logs matter
Small-business AI failures are often small enough to ignore: a wrong appointment detail, an invented policy phrase, a missing source note, a tone mismatch, or a customer record summarized too confidently. Without a log, the team only remembers that “AI needed editing.” With a log, the owner can identify whether the issue is the prompt, the source data, the tool setting, the review rule, or the workflow itself.
Copy/paste AI output correction log
| Field | What to capture | Stop rule |
|---|---|---|
| Date / workflow / owner | Workflow name, tool, prompt version, reviewer, and who owns the fix. | No owner means no expansion. |
| Original output | Paste the exact AI output or summary that needed correction, with any customer/private details redacted when sharing internally. | Do not invent what the AI said; cite the actual output. |
| Correction type | Wrong fact, unsupported promise, missing context, stale source, privacy issue, bad tone, policy risk, handoff miss, or too much review effort. | Policy, price, legal, medical, safety, refund, warranty, or privacy issues require manager review. |
| Source of truth | Link or paste the approved record, SOP, CRM note, policy page, invoice, customer file, or manager decision used to correct it. | No source, no customer-facing correction. |
| Customer / staff impact | Who saw it, what confusion or rework happened, and whether a correction/apology/follow-up is needed. | Customer-facing impact needs an owner-approved response. |
| Prompt or workflow fix | Prompt change, source-data change, tool setting, checklist update, reviewer rule, fallback path, or pause decision. | Do not claim the workflow is fixed until new examples pass review. |
| Retest evidence | Sample size, date range, before/after examples, reviewer notes, and remaining risks. | One clean demo is not proof for live expansion. |
Reusable owner-review prompts
Act as a cautious AI workflow reviewer for a small business. Use only the correction-log facts below. Do not invent customer impact, approval status, policy meaning, legal advice, time savings, or root cause. Return: 1) missing evidence, 2) likely root cause categories, 3) customer/staff risk, 4) whether to keep/fix/pause/expand, 5) required prompt/source/workflow changes, and 6) STOP AUTOMATION triggers before reuse.
Correction log:
[Paste rows here]Draft an internal correction note for the owner to review. Include: what happened, source of truth, customer/staff impact, correction made, prevention step, retest plan, and next review date. Do not blame a staff member, hide uncertainty, or promise the workflow is fixed without retest evidence.Safe correction message snippets
Internal note: We corrected [workflow/output] because [verified issue]. Source checked: [source]. Owner: [name]. Prevention step: [prompt/source/review change]. Retest by: [date]. Do not expand this workflow until [evidence required].
Customer-facing correction draft for manager review: Hi [Name] — I want to correct one detail from our earlier message. The verified information is [correct fact/source]. Sorry for the confusion. We will use [next step] from here. [Owner/team] is reviewing the workflow so this does not repeat.
Pause decision: Pause [AI workflow] for [use case] until [owner] verifies [source data], updates [prompt/review rule], and reviews [sample size] new outputs without the same correction type.
Fast QA before reusing the workflow
- Can a reviewer point to the source of truth for every corrected claim?
- Did the fix reduce review effort, or just make the prompt longer?
- Does the workflow now stop before customer-facing promises, privacy details, policy exceptions, pricing, refunds, warranties, legal wording, or safety-sensitive claims?
- Was the same mistake found in any already-sent customer messages, saved templates, SOPs, chatbot answers, or knowledge-base entries?
- Is there a fallback manual process if the workflow must pause?
