AI incident triage checklist

  1. Pause the workflow: stop further sends, uploads, automations, or reuse of the prompt/output until reviewed.
  2. Save the evidence: original input, AI output, tool name, date/time, user, source document, destination, and who saw it.
  3. Classify the issue: wrong facts, wrong customer, private data, tone/brand issue, policy/pricing claim, unsafe advice, hallucinated source, or skipped review.
  4. Name the owner: assign one person to decide correction wording, customer/internal update, and restart rules.
  5. Correct the record: update CRM, document, website, email draft, ticket, or customer note with the verified correction.
  6. Notify only when useful and truthful: do not overstate certainty, blame, severity, or legal conclusions.
  7. Fix the workflow: improve the prompt, remove risky data, add missing source fields, require approval, or retire the workflow.
  8. Restart only after review: document the new rule and test one safe example before team-wide use.
