1. Action item

Field: What needs to happen? Write the smallest concrete next step, not a broad project name.

Example: Send revised estimate with warranty note; confirm vendor arrival window; update FAQ page source link.

2. Owner and backup

Field: Primary owner: [NAME/ROLE]. Backup owner: [NAME/ROLE]. If no owner was named, write UNASSIGNED — needs owner review.

Human review rule: unassigned items do not go to customers, project boards, or CRM automations until a person accepts ownership.

3. Due date and trigger

Field: Due by: [DATE/TIME]. Trigger: [customer reply, invoice paid, vendor confirms, owner approves, next weekly review]. If no timing was stated, write NO DEADLINE STATED.

Risk note: AI should not infer urgency from tone. Use only explicit deadlines or owner-approved priority rules.

4. Source note

Field: Evidence from notes: [paste the sentence, bullet, transcript timestamp, or source document that supports this action].

Human review rule: every customer-facing action needs a source note before it is sent.

5. Customer/team follow-up

Field: Follow-up needed: [none/email/text/call/task]. Draft message: [copy]. Review flag: [price/date/policy/promise/private info/manager approval].

Stop rule: if the message contains price, discount, warranty, legal, medical, safety, refund, or availability claims, require owner approval.

6. Closeout status

Field: Status: [open / waiting / done / blocked / cancelled]. Closeout proof: [sent message, CRM update, customer reply, file link, owner approval].

Weekly review: look for repeated blockers and add them to a process improvement list.
